Any guilds that exist in the anime still have to fill this out, (Which means Fairy Tail, Sabertooth, ETC!)
^ Though for existing guilds in the anime, anyone of the guild members can fill it out. (As in Natsu can fill it out for Fairy Tail if he wants to)
But they have to put the real guild master as the guild master. (If Natsu fills this out for Fairy Tail he must put Makarov as the guild master.
And for the rules, if you don't know any rules for this said existing guild in the anime .. feel free to make up some.
